NO REFUNDS
All sales are final and will not be refunded; including portions of unused packages, pre-paid memberships, and missed, unattended classes, workshops, events, or private appointments. 

 
PAYMENT PROCESSING
All purchases will have payment processing fee assessed at checkout. 

EXPIRATION DATES
Purchases are valid for a predetermined length of time. Expiration dates on purchases ensure that the item is used within a specified time frame so in accordance with its current value. Expiration dates may not be revised, modified, or placed on hold. Unused portions of any purchase are not redeemable beyond expiration dates.

PACKAGE EXTENSIONS
A one-time extension may be purchased for $15 and applied to expired Class Passes. Expiration date will be revised to 30-days from date of purchased extension. Extensions are not available on unlimited package options, workshops, events, or private appointments.  
 
NON-TRANSFERABLE
Purchases are redeemable only for specified purchase. Dollar values may not be transferred and applied to other services or products.

MAY NOT BE SHARED
Purchased items are only redeemable by the individual with the purchase on their account. Class passes, workshops, events, or appointments may not be gifted or shared with third party individuals.

TRANSFER OF OWNERSHIP
A one-time transfer may be purchased for $30 to transfer ownership of any active Class Passes to a third party. Expiration dates will not be revised with a transfer of ownership. Transfers are not available on unlimited package options, workshops, events, or private appointments.

PURCHASE COMPATIBILITY
Please note that not all promotional offers may be compatible with existing purchases. When purchasing multiple offers please be advised that all purchases are utilized in the order they are purchased and not in accordance with purchase expiration dates. Sorry, no exceptions.

STUDIO UPDATES
Elevate Yoga & Wellness reserves the right to modify class schedules, class times, class styles and associated staff. Refunds or partial refunds are not available due to any type of studio update.

RETAIL PURCHASES 
All sales are final. Products purchased from Elevate Yoga & Wellness's retail location are not eligible for return unless they are returned to the studio within 7-days from day of purchase with valid purchase receipt. All items must be unused, unwashed and undamaged. Elevate Yoga & Wellness reserves the right to refuse a requested return.

TEACHER TRAINING
An applicant who provides written notice of cancellation within three (3) days (excluding Saturday, Sunday, and federal or state holidays) of purchase is entitled to a refund; minus the processing fees.

Booking & Appointment Policies

BOOKING POLICY
Bookings are required to secure attendance for all classes, events, workshops, and appointments. Walk-ins are welcome but space cannot be guaranteed, regardless of package, without pre-booking. Reservations can only be made with a pre-purchased booking. Unpaid reservations will not be accepted. 
 
THIRD PARTY BOOKINGS
Bookings cannot be made in the name of a third party. Each attendee must reserve individually for any specific class, workshop, event, or appointment. 

 
BOOKING TRANSFERS
Bookings may not be transferred to the name of a third party. Bookings will only be accepted by the originator of the booking. 

 
BOOKING FORFEIT
Bookings will only be held until class / appointment start time; at which time reservations will be forfeited and given away to wait-listed guests. Missed Bookings policies will still apply. 


ARRIVAL TIME
Please arrive ten (10) minutes prior to all scheduled appointment times to allow ample time to check-in and complete any necessary paperwork. Arriving at appointment start time will shorten allotted scheduled appointment time.

APPOINTMENT TIMES 
All appointments begin and end promptly on time. Any late arrivals will impact treatment time length; no exceptions.
 
MISSED BOOKINGS
Missed bookings are any reservations made with failure to attend. Missed classes / appointments do forfeit the pass or purchase utilized to secure the booking. Memberships and Class Pass holders will accrue a $10 Late Cancellation Fee. 
 

EARLY CANCELLATIONS 
Early cancelled bookings are any reservations cancelled four (4) or more hours prior to class / appointment start time. Early Cancellations do not accrue a Late Cancellation Fee.  Early cancellations may be completed by client by logging into individual account and removing yourself from the reserved class / appointment. Early cancellations may not be requested via emails or voicemail. 
 
LATE CANCELLATION 
Late cancelled bookings are any reservations cancelled within four (4) hours of class / appointment start time. Late cancellations do forfeit the pass or purchase utilized to secure the reservations. Memberships and Class Pass holders will accrue a $10 Late Cancellation Fee.

Member Policies

INTRODUCTION
Memberships at Elevate Yoga & Wellness include: Casual Flyers, Frequent Flyers, and VIP Flyers. All Membership Plans, Membership Agreements, the Studio Rules and Regulations may be modified from time to time at the sole discretion of the Elevate Yoga & Wellness.
 

STUDIO FACILITIES 
Elevate Yoga & Wellness offers levels of membership that enable access to the studio's unique Aerial Yoga Classes, Meditation, Special Events, Workshops, Specified Training, and Social Activities. Additional costs may apply to specified activities. 


STUDIO PRIVILEGES 
Elevate Yoga & Wellness offers a variety of membership options that include specific privileges associated with each category of membership are are more fully described in the Membership Plans. Elevate Yoga & Wellness may offer certain other memberships and use privileges as described.

NON-EQUITY MEMBER
Elevate Yoga & Wellness maintains all facilities and all major decisions regarding the operation;  including management, staffing, hours of operation, policies, programming, and maintenance and improvements. Elevate Yoga & Wellness Members are responsible only for membership monthly dues and any additional charges the individual member may accrue. Memberships are non-equity and members therefore are not subject to either operating or capital assessments.

MEMBER ADVANTAGES
Member advantages are non-transferable and are only intended for use by the enrolled member. Any services rendered under false pretenses will result in the immediate termination of said agreement.
 
MEMBER CONTRACT
Elevate Yoga & Wellness Membership dues are on-going with automatic renewal payments deducted each month until client cancels membership by providing at least a 30-day notice prior to automatic billing date. Unused portions of monthly memberships do roll into the following month. Memberships auto-renew until notice of cancellation is provided.
 

AUTOMATIC RENEWAL
Elevate Yoga & Wellness Membership rates will be automatically charged every 30-days on the renewal date (billing date) by client authorized automatic credit or debit card, charges or automatic draft from a checking account. It is the member’s responsibility to keep account information current and up-to-date; any changes must be reported immediately to avoid cancellation. Any payments in increments by any means other than client credit or debit card is not permitted (no cash / no checks).

PAYMENT DEFAULT
Any defaults or declined charges, for any reason, a $15 decline charges fee will be applied to member's account. If valid billing information is not provided within three (3) days of the rejection date member's services will be deactivated. Memberships cancelled due to defaults, or declined charges, are still liable for the $15 declined charge fee. If valid billing information is not provided within fourteen (14) days of the rejection date may result in automatic suspension of membership privileges. Member agrees to pay any collection and / or legal costs incurred by Elevate Yoga & Wellness for collection of any amounts due.  Member authorizes Elevate Yoga & Wellness to charge any undisputed balances and overdue amounts to your then current payment method on file.

NO REFUNDS
Membership dues collected per the membership contract agreement are non-refundable. Elevate Yoga & Wellness is not liable for mismanaged member accounts and is unable to provide refunds on any charges if the proper termination procedures are not followed. Membership dues may not be refund or prorated to accommodate unused portions or member loss of time. 

CANCELLATION NOTICE
Elevate Yoga & Wellness memberships may be cancelled by members by submitting the required 30-Day Notice of Cancellation to the studio via the online cancelation form. This form can be requested by contacting Elevate Yoga & Wellness. Members are fully responsible for cancelling their membership by submitting the required notice 30-days prior to their scheduled billing date. Memberships cancelled after scheduled billing date, or less than 30-days prior to scheduled billing date, are not entitled to a refund of charges. Cancellation of any membership (e.g., 6-month, 12-month, etc.) is permitted under certain circumstances and may be subject to a cancelation fee (cost will be calculated based on the number of classes that were attended at the current drop in rate), unless where required by applicable law, which may include permitted cancellation in the event of death, disability or moving more than 25 miles from ElevateYoga's studio, in which event the cancelation fee is not applicable. Members may also opt to transfer membership to another person to avoid cancelation fee. Elevate Yoga & Wellness is not liable for mismanaged accounts by Members. 
 
MEMBER FREEZE NOTICE
Elevate Yoga & Wellness Memberships may be frozen by members by submitting the required 30-Day Notice of Freeze in the studio. Members are fully responsible for freezing their membership by submitting the required notice 30-days prior to their scheduled billing date. Memberships may only be frozen once in a 12 month timeframe for up to 3-months. Elevate Yoga & Wellness is not liable for mis-managed accounts by Members. ​


MEMBER RESERVATIONS
Elevate Yoga & Wellness Memberships are eligible to make advanced reservations for all studio classes, events and workshops. 
 

MISSED BOOKINGS
Missed bookings are any reservations made with failure to attend. Members reserving classes agree that missed reservations will result in a $10 Late Cancel Fee, applied directly to their billing information on file, for each missed pre-registered class not cancelled within the four (4) hour allotted cancellation window.
 

LATE CANCELLED RESERVATIONS
Late cancelled reservations are any reservations cancelled within four (4) hours of class / appointment start time. Late cancellations do forfeit the pass or purchase utilized to secure the reservations. Memberships and Class Pass holders will accrue a $10 Late Cancellation Fee.

ADDITIONAL CHARGES 
Elevate Yoga & Wellness Members authorize studio staff to charge their accounts directly for any additional services, fees, purchases, guest admittance, specialty workshops, training, or event that members may opt to utilize.

Liability Policies

Elevate Yoga & Wellness Participants Agree & Acknowledge

PARTICIPATION STATEMENT
In consideration of participating in Yoga and aerial Yoga with Elevate Yoga & Wellness, LLC (“Activity”), I represent that I understand the nature of Yoga and aerial Yoga and that I am qualified, in good health, and in proper physical condition to participate in such Activities. I fully understand that such activity involve risks of serious bodily injury, including permanent disability, paralysis and death. I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the Activity. 

CLAIMS
I hereby release, discharge, and covenant not to sue Elevate Yoga & Wellness, LLC, its respective administrators, directors, agents, officers, volunteers, instructors, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises (“Releasees”) on which the Activity takes place from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations; and I further agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the releasees from any loss, liability, damage, or cost which any may incur as the result of such claim. 
 
INJURY

I have read this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, AND PARENTAL CONSENT AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it be a complete and unconditional release of all liability to the greatest extend allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

PHOTOGRAPHY
I further give permission for images of myself, captured during regular and / or special events, to be solely used for the purposes of promotional material and publications, both online and in print format, and waive my right to compensation or ownership thereto.

REIKI CONSENT
I understand that Reiki is a simple, gentle, hands-on energy technique that is used for stress reduction and relaxation. I understand that Reiki practitioners do not diagnose conditions nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. I understand that Reiki does not take the place of medical care. It is recommended that I see a licensed physician or licensed health care professional for any physical or psychological aliment I may have.
 
I understand that Reiki can complement any medical or psychological care I may be receiving. I agree that I will inform my practitioner of any significant health changes prior to future appointments. I also understand that the body has the ability to heal itself and to do so, complete relaxation is often beneficial. I acknowledge that long- term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself. I understand that Reiki practitioners are providing Reiki at my request and are not responsible for the outcome of the session. Reiki treatments are given fully clothed on a massage table. Blankets, bolsters and soft music may be used for additional comfort or relaxation benefit.

PRIVACY STATEMENT
At Elevate Yoga & Wellness the privacy and security of your personal information and billing information is very important to us. Whether you are an active member, previous member, an email subscriber, or an occasional visitor, you can trust that the information that you have provided to us is being properly managed and protected. Information is not shared nor sold. Elevate Yoga & Wellness is committed to collecting and using your information in accordance with applicable data protection laws.

HOW INFORMATION IS USED
Elevate Yoga & Wellness collects and use personal information if you make a purchase, reservation, participate in an event, or enroll as a subscriber.  Information is collected directly from clients, or is submitted by existing clients to direct friends. We collect information through our online booking and studio management software and our online subscriptions. Information is used to inform clients of current and upcoming events, promotions, studio updates and personal client contact needs.

UNSUBSCRIBE
Elevate Yoga & Wellness clients are required to disclose all personal information on their liability forms but are not required to maintain an active subscription to studio emails and marketing. Clients are not required to remain subscribed; however, unsubscribing from studio notifications will deactivate your account and limit a client's ability to make future reservations or participate in loyalty program.
